Do we need to change our discharge workflow? +
No. We listen for your existing discharge event and work from your existing chart. There's no new form for your nurses to fill out, and the patient experience looks like a call from your care team.
What languages are supported? +
English and Spanish in production. Additional languages ship quarterly based on customer demand.
Who owns the navigator staffing? +
Rely. Navigators are our W-2 employees, trained on your clinical protocols, and dedicated to your program. You don't staff or supervise them.
What happens if the patient doesn't answer? +
The AI retries over a compliant cadence. After three attempts across channels, the case is escalated to a human navigator to track down. We report unreachable patients back to you weekly.
